This includes West Virginia State Agencies, County Agencies, and Local Government Agencies. (This does not include Federal Agencies.) In order to signup, you must complete all three forms below. Once completed, your information will be reviewed and processed. Applications must be approved by WVDMV before access is granted to the system.
