UCC-1

New Filing

A UCC-1 is a legal form that a creditor files to give notice that it has or may have an interest in the personal property of a debtor.

UCC-3

Assignment / Amendment / Continuation

A UCC-3 Amendment is a legal form created for changing or adding information contained in the UCC-1 Form.
A UCC-3 Assignment is a form used to assign the rights to the collateral mortgage to another party.
A UCC-3 Continuation is an amendment form to UCC-1 used to extend the UCC-1 statement's effectiveness.

Termination

A UCC-3 Termination is a form used to end a lender's interest in the collateral mortgage.

UCC-5

Statement of Claim

A UCC-5 Information Statement may be filed by any authorized parties, if it is believed that a particular record is inaccurate, was wrongfully filed, or filed by an unauthorized party.

Most documents maintained by the Secretary of State’s Office are public records and are subject to disclosure. Filings may be viewed by the public and requested copies may also be made available.

Please do not submit Personal Identifiable Information (PII) unless you are specifically asked to provide it. The Secretary of State’s Office only collects personal identifiable information in a very limited number of cases and will specifically ask for it when it is required. PII includes your social security number, driver’s license number, state-issued ID number, credit or debit card number, or tax ID number on a UCC filing or any other documents you submit to the Secretary of State.

Although we attempt to prevent disclosure of personal identifiable information, due to the large number of documents filed, we cannot guarantee that if PII is placed on a document it will not be disclosed. It is the responsibility of the filer to ensure that PII is not contained on the filing and to notify the office for removal should any be found.
